The movie begins with a flashback to 2019, when Josh Lambert (Patrick Wilson) was possessed by the spirit of the Bride in Black after returning from The Further, a dark realm where the souls of the dead linger. He killed his mother Lorraine (Barbara Hershey) and tried to kill his wife Renai (Rose Byrne) and his son Dalton (Ty Simpkins), who also has the ability to astral project into The Further. Josh was stopped by Carl (Steve Coulter), a friend of Lorraine and a fellow traveler, who used hypnosis to make Josh and Dalton forget their experiences in The Further.
Ten years later, Josh is divorced from Renai and estranged from Dalton, who is now a college student. Josh has repressed his memories of The Further, but he begins to see visions of his father Ben Burton (Peter Dager), who he believed abandoned him when he was young. Ben reveals that he was also a traveler and that he was killed by a demon called Smash Face (David Call) when he tried to rescue Josh from The Further as a child. Ben warns Josh that Smash Face is still after him and that he needs to go back to The Further to face him.
Meanwhile, Dalton attends an art class where he draws a picture of the red door, the entrance to The Further. He also meets Kali (Juliana Davies), a mysterious girl who seems to know about his abilities. Dalton goes to a frat party with his roommate Chris (Chris Winslow), where he sees the ghost of a student who died of alcohol poisoning. He accidentally astral projects into The Further, where he encounters Elise Rainier (Lin Shaye), a psychic and an old friend of Lorraine, who died in the first movie. Elise tells Dalton that he needs to help his father and that she will guide him through The Further.
Dalton contacts Specs (Leigh Whannell) and Tucker (Angus Sampson), two paranormal investigators who worked with Elise, and asks them for help. They show him a video of Elise explaining how The Further works and how to travel there safely. They also tell him that they have been tracking down other travelers who have been targeted by Smash Face and his minions, such as Alec Anderson (Justin Sturgis), a hacker who used his abilities to expose corruption and crime.
Dalton decides to go back to The Further with Specs and Tucker’s assistance. He finds Josh in an abandoned hospital, where Smash Face has trapped him in a loop of his childhood memories. Dalton tries to free Josh, but Smash Face attacks them. They manage to escape through the red door, but they end up in a different part of The Further, where they meet Professor Armagan (Hiam Abbass), an expert on ancient civilizations and occultism. She tells them that Smash Face is actually an ancient entity that feeds on the souls of travelers and that he has been collecting them for centuries. She also reveals that Kali is her daughter and that she is also a traveler.
Armagan explains that there is a way to stop Smash Face: they need to find the Eye of Horus, an ancient artifact that can seal him away. She says that the Eye is hidden in a pyramid in Egypt, but they need to find four keys to open it. She gives them the first key, which she obtained from Alec before he was captured by Smash Face. She also warns them that Smash Face has allies in the real world, such as Nick the Dick (Nick Offerman), a corrupt politician who uses his influence to cover up Smash Face’s activities.
Dalton and Josh embark on a perilous journey through The Further, where they encounter various horrors and challenges, such as the Lipstick Demon (Joseph Bishara), the Long-Haired Fiend (Javier Botet), and the Sick Kid (Stephen Gray). They also meet other travelers who help them along the way, such as Sorority Girl #1 (Bridget Kim), who gives them the second key, and Una Experiencia Aterradora (Eduardo Noriega), who gives them the third key.
Meanwhile, Renai becomes suspicious of Nick’s involvement in Josh’s disappearance and decides to investigate him with the help of Carl. They discover that Nick is working with Smash Face and that he has kidnapped Kali, who he plans to use as bait for Dalton and Josh. Renai and Carl manage to rescue Kali and bring her to Specs and Tucker’s headquarters, where they join forces with them.
Dalton and Josh finally reach the pyramid, where they find the fourth key guarded by Smash Face himself. They engage in a final confrontation with him, during which Josh sacrifices himself to distract Smash Face while Dalton opens the Eye of Horus. Dalton activates the Eye, which emits a powerful light that banishes Smash Face and frees all the souls he had captured. Dalton returns to his body in the real world, where he is reunited with Renai, Kali, Specs, Tucker, Carl, and the other travelers. They mourn Josh’s death, but they also celebrate their victory over Smash Face.
The movie ends with a scene of Elise in The Further, where she meets Josh’s spirit. She tells him that he is a hero and that he can rest in peace. She also says that she has a message for him from Lorraine: she loves him and she is proud of him. Josh smiles and hugs Elise, as they fade away into the light.
